How IDS Supports Community

Our department positively impacts Fresno County schools and their community partners by providing the infrastructure and support needed to use data in an actionable way. This cross-sector integrated system will help small school districts, and others, that can’t currently turn a variety of data points into information that can help drive valuable improvements. Currently, a number of schools, government agencies, and health and mental health care providers in addition to local non-profit agencies serve Fresno’s children and their families, yet many are not receiving the support they need while others might receive duplicate services. Including Cradle to Career, we will help all leaders and policymakers better understand the needs of Fresno’s children and families while supporting continuous program improvements, innovation, policy, and research.

care coordination

Care Coordination and the Community Information Exchange

Integrating person-level data across the education, health, and social services sectors to better understand the “whole person” makes it possible to support needs effectively. The goal in Phase I of this project is to ensure that an appropriate referral is made and the person/organization who made the referral knows that the needed service is in process.

Unique ID

Developing and Utilizing Unique Resident ID

By creating shared unique IDs using matching strategies applied to individual level data we can maximize coordination, streamline access for students and families and understand how education and workforce pathways are operating.

Dual Enrollment

K-16 Collaborative Dual Enrollment Project

The K-16 Collaborative is part of the Developing the Region’s Inclusive and Vibrant Economy (DRIVE) Investment Plan and is situated in the human capital part of the DRIVE portfolio. Economic mobility generated by post-secondary enrollment and completion of degree or certification programs is central to the goals of DRIVE and priorities of the K-16 Collaborative.

Broadband

This cross-sector collaborative includes K-12, Higher Education, Government and Community-Based Organizations. The collaborative is an official workgroup under the San Joaquin Valley Regional Broadband Consortium sponsored by the Office of Community and Economic Development at Fresno State.
